Our client plays a vital role in the community, supporting older Australians to live independently through flexible, person-centred home care tailored to individual needs and real-life routines.

The Role:
As a Service and Care Coordinator, you will take ownership of scheduling and rostering to ensure services are delivered efficiently, consistently, and on time. This role is centred around building and managing effective rosters, matching the right support workers and nurses to customers, and ensuring continuity of care through proactive schedule management.

You will coordinate services in line with individual care plans, conduct welcome calls to understand availability and preferences, and translate these into structured, sustainable rosters. Posting job requests, allocating shifts, confirming bookings, and maintaining ongoing schedules will form the core of your day-to-day responsibilities.

A key focus of the role is managing changes --- from last-minute cancellations to urgent care requests --- while minimising disruption to customers. You will liaise regularly with customers, families, and service providers to confirm availability, adjust schedules, and ensure seamless service delivery. Maintaining accurate records, meeting service timeframes, and ensuring compliance standards are met will also be essential.

This is a fast-paced coordination role requiring strong time management, attention to detail, and the ability to balance multiple scheduling priorities at once.

About You:

You bring experience in rostering, workforce scheduling, service coordination, or customer operations. You are highly organised, solutions-focused, and confident managing competing priorities in a dynamic environment.
You communicate clearly with both customers and service providers and are comfortable working across systems to manage bookings, rosters, and administration efficiently.

What’s On Offer:
  • $75,000 + super
  • Hybrid working (2 days in the office, 3 days from home)
  • Rotating roster between 7am–7pm (7.6 hours per day)
  • Occasional Saturday work (rotated)
  • 2 weeks paid training with strong onboarding and support
